2024/25 Membership Form

You are invited to become a member of ConnectGV for financial year 2024/2025.

The principal objective of the Company is to undertake public charitable activities in Australia to advance social or public welfare and education in the field of disability.

The following people may apply for membership of the Company:

  • a) parents, guardians and friends of any person attending or using the facilities of the Company or on the waiting list; or
  • b) any interested citizen who applies for and is granted membership of the Company.
  • Membership of the Company is conditional on payment of an annual subscription fee determined by the Board. The Board may increase the annual subscription fee from time to time, applicable from July. The current Membership cost is $2.00. Payment can be made by Direct Debit into ConnectGV’s Bank Account (details below), or by cash payment at reception; receipts for Direct Debit payments will be emailed out to members.

    Please return the completed Membership Application form (on following page) asap, via email to ceo@connectgv.com.au or to reception at 12 Bowenhall Street, Shepparton.

    The 2024 Annual General Meeting will be held on 30th October; paid Membership is necessary to ensure your eligibility of voting rights at the AGM.
