ConnectGV’s Individual Support Program, Employment and Accommodation are offered by talented and qualified support staff. For 70 years ConnectGV has been educating the community about people with a disability through participation in community based activities and employment options.
Day Programs
ConnectGV’s day programs enable participants to maximise opportunities to increase their skills to transition through various stages to reach their goals, in educational, vocational or community and social. While encouraging a high degree of autonomy as they take an active role in planning their futures. Participants learn practical skills that they can use in their community every day.
Individual Support
ConnectGV Individual Support offers tailored services to people with disabilities.
Our staff support people with a disability in a dignified and respectful manner in their home and within the community to achieve their goals.
The Individual Support Program assists clients with completing daily living tasks, accessing the community, social inclusion and building skills to live independently.
Supported Employment
ConnectGV offers Supported Employment funded by the NDIS, for local people with a disability through our Social Enterprises.
Supported Employees are employed in meaningful work and have the opportunity to further develop their skills to achieve their individual employment goals.
Accommodation
ConnectGV own several purpose-built houses to provide supported accommodation and independent living options. Our houses are located close to shops and community facilities which enable increased opportunities and independence whilst also providing individualised support.
To be eligible for accommodation, a goal for living independently must be identified in your NDIS plan and a home and living assessment is required from your Occupational Therapist.
Plan Management
ConnectGV is a registered Plan Management provider. The role of the plan manager is to ensure your NDIS plan is implemented as intended.
This includes ensuring funds are being spent in accordance with your plan, advising on over or underutilisation of your funds, and providing financial reporting to the NDIA.
